The loan application checklist
After you have contacted First Choice of Maine and you want to apply for a loan, it’s best to start putting together the following information. The information listed below is necessary to go further in the loan process.

Last 2 year’s W-2 form or last 2 year’s Federal Tax Returns (1040), including all schedules
2 most recent pay stubs
A copy of your most recent tax bill (may be provided by the town office)
A copy of your deed
A copy of your home owners insurance policy

If you are self-employed:

Last 2 year’s professionally prepared Federal Tax Returns (1040), including all schedules
Last 2 month’s bank statements
A copy of your most recent tax bill (may be provided by the town office)
A copy of your deed
A copy of your home owners insurance policy

Depending on your individual situation, you may be required to submit some additional information.

If you own other real estate, provide lease/rental agreement if applicable, and copies of property tax and insurance bills.
If you receive alimony or child support, provide a copy of divorce decree and evidence of receipt of income for 12 months (i.e. canceled checks or bank statements)
If you pay alimony or child support, provide a copy of your divorce decree.
If you have had a bankruptcy in the last seven years, provide a copy of your bankruptcy discharge.
